To ALL Eden/Genesis Employees:
We are pleased to announce the 2012 Health Reimbursement Account (HRA). The HRA provides money for your health care costs that are not reimbursed to you under other benefits plans.

To be HCA eligible you must:

• be benefits eligible (80% full time equivalent, or greater) in any department, and
• be hired no later than September 30, 2011

If you can answer “yes” to both 1 and 2 above, you may be eligible to receive:

• A full HRA share, if your Home Department is in an OMRDD/DMH Residential, Adult Day, or Family Services program
• A fixed percentage of a full HRA share, if your home department is in Central Administration

A portion of a full HRA share, determined by the number of OMRDD/DMH hours you worked in the prior year, if your Home Department is in one of our School or Outreach programs, and you also averaged at least 10 hours per week in an OMRDD/DMH funded/certified program during calendar year 2011.

However, regardless of whether you otherwise are eligible, you must apply to participate in this new HCA program. After you complete and return the application, you will be advised (in writing) whether or not you qualify, and if you do qualify, the dollar amount available to you. If you have any questions about this program, please contact your supervisor, or any member of the HR department.

2011 Health Reimbursement Account (HRA)

A Lump Sum Health Care Supplement

• Result of our advocacy with OMRDD
• A lump sum is placed in a Health Care Reimbursement Program (HRA) set up in your name for any year which you enroll
• The HRA can be rolled over into future years if not needed immediately

OK, But What is it Exactly?

• The HRA provides a lump sum to eligible employees who enroll in the plan for medical/health expenses not otherwise covered by health insurance.

Am I Eligible?

You are eligible for the HRA if

• You were hired no later than September 30, 2009.
• You are eligible for benefits (80% full time equivalent, or greater) in any department.

How much am I eligible to receive?

If you meet the eligibility requirements above, you may be eligible to receive:

• A full HRA share, if your Home Department is an OMRDD/DMH Residential, Adult Day, or Family Services program.
• Approximately 49% of a full share, if your Home Department is in Central Administration.
• A portion of a full HRA share, determined by the number of OMRDD/DMH hours you worked in the prior year, if your Home Department is in one of our School or Outreach programs, and you also averaged at least 10 hours per week in an OMRDD/DMH funded/certified program during the calendar year 2009.

What Can I Use My HRA For?

You can use the money in your HRA to pay expenses that are not covered, or partially covered, by your health insurance, as follows:

• Medical expenses
• Dental expenses
• Vision expenses
• Drug co-payments
• Some over-the-counter medications

The HRA can only be used for qualified expenses.
