Long-Term Disability Benefits
Eligible salaried Eden II employees are offered group long-term disability coverage on a non-contributory basis as part of their overall Eden II School benefits package once they have completed three months of full time employment. The following disability information is only a summary of the group long-term disability benefits you will receive as an eligible salaried Eden II employee.

Long-Term Disability Plan Features:
- Monthly Benefits Percentage: 60% of Salary
- Maximum Monthly Benefit: $6,000
- Minimum Monthly Benefit: Greater of $100 or 10% of Gross Benefit
- Annual Benefit Adjustment: Equal to Full Chang in CPI Not to Exceed 3%
- Increasing Monthly Wage Base: Compounded Annually at 3%
- Waiting Period: 3 Months after reported disability
- Elimination Period (if a pre-ex does not apply): 3 Months
- Elimination Period (if a pre-ex limitation does apply): The longer of
- (a) 12 months period beginning on the date you became insured under the program
- (b) 3 months
- (c) Any period you are eligible to receive leave or a salary continuation plan where a monthly wage base equivalent is being received periods a, b, or c will run concurrently
- Return to Work During Elimination Period: 90 day leeway
- Work Transition Benefit: 12 Months
- Recurrent Disability: 6 Months
- Normal Occupation Period: 2 Year Own Occupation
- Survivor Benefit: 3x Last Monthly Benefit Paid in Lump Sum
- Rehire Provision: 90 Days
- Leave of Absence Provisions: Up to 2 Years Depending on Reason for Leave
- Integration: Full Family
- Mental/Nervous Limitation: 24 Month Limitation
- Conversion Privilege: Yes to Max Benefit of $4,000

- Less than 60 – to age 65
